Abstract
High-risk prostate cancer is a heterogeneous and aggressive disease, associated with increased rates of biochemical recurrence, local relapse, distant metastases, and mortality. Accumulated data from clinical trials, including the STAMPEDE study, demonstrate a significant benefit from adding Abiraterone to long- term androgen deprivation therapy (ADT) following definitive radiotherapy in patients with localized or locally advanced very high-risk prostate cancer. The combined treatment leads to a marked improvement in metastasis-free survival (MFS), progression-free survival (PFS), and overall survival (OS). Based on these results, leading international guidelines (NCCN, EAU, ESMO) now recommend Abiraterone as part of standard therapy. Despite its established role in the therapeutic algorithm, full access and reimbursement for this indication are still lacking in Bulgaria.
