Total hip arthroplasty after surgically treated acetabular fractures

Abstract

Introduction: Acetabular fractures are complex injuries that frequently affect young patients following high-energy trauma, as well as elderly individuals with osteoporotic bone after low-energy mechanisms. Despite advances in open reduction and internal fixation (ORIF) techniques, post-traumatic complications such as osteoarthritis, avascular necrosis of the femoral head, and heterotopic ossification remain common. These sequelae often result in persistent pain and functional impairment, ultimately necessitating conversion to total hip arthroplasty (THA).

Methodology: We present 11 patients who underwent THA following prior ORIF of an acetabular fracture between the years 2019 and 2024. Mean age at THA was 59 years(range 48 to 72). 8 patients were male. 2 patients were implanted patient specific custom acetabular implant. 3 patients were implanted stemmed acetabular component. 2 patients were implanted cemented acetabular components. 1 case had Kerboull device. 3 patients were implanted cementless acetabular cups with or without screws. Hardware from previous fixation was removed in case it interferes with acetabular reaming and implant positioning.

Results: The time range from fracture to THA was from 8 to 312 months. Mean follow-up after the THA was 3 years(1 to 5 years). The mean preoperative HHS score was 43(range 24 to 56) and 90 after arthroplasty(range 82 to 96). The most prevalent postoperative complications were heterotopic ossification-2 cases, infectione-1 case.

Conclusion: Total hip arthroplasty following acetabular fractures represents a complex reconstructive challenge that aims to restore hip biomechanics alleviate pain and improve patient function. Meticulous preoperative planning, precise surgical execution and diligent postoperative rehabilitation are essential to optimize outcomes and minimize complications.
