Laparoscopic treatment of Vesicoureterorenal reflux (VUR ) using Lich-Gregoir technique in pediatric patients

Abstract

Introduction: Vesicoureterorenal reflux (VUR) is one of the most common congenital urological anomalies, with an incidence of ~1%. Different surgical techniques are used in pediatric patients – endoscopic, conventional, laparoscopic and robot-assisted. Laparoscopic Lich-Gregoir reimplantation is a technique which gives good results with a low incidence of postoperative complications, shorter hospital stay and less trauma.

Objectives: The aim of this case report is to present the technique used and reveal the initial results of three laparoscopic Lich-Gregoir reimplantations.

Materials and methods: From June to December 2024, we operated on five patients with vesicoureterorenal reflux (VUR). Four of them were with unilateral form, and the other one was with bilateral form. The average age of the operated patients was 3 years

Results: We performed laparoscopic Lich-Gregoir reimplantation using the described technique in all patients. Surgical time – 1 h 50 m. The ureter was not intubated with a stent, the urethral catheter was removed 12 hours postoperatively in one case and 24 hours postoperatively in the other four cases. 5-day antibiotic treatment was applied. In the first month, hydronephrosis 1st grade was present in two of the patients.

Discussion: Laparoscopic Lich-Gregoire reimplantation in kids with reflux VUR is an alternative to open surgical techniques, giving identical postoperative results and being less traumatic. Performing this technique significantly reduces hospital stay, does not require ureteral stent placement and reduces infection rates and hematuria due to shortening of the urethral catheterisation period.
